Visitors:

Visitors to our school: All people who enter the school building between the hours of 9:00 and 3:00 must obtain a visitor’s pass at the security desk that is located at the main entrance on Prairie Ave.

School Hours:

Grade- Pre-K:  8:20 – 11:30 (Fee supported program runs from 11:30 to 1:50) 

Grades- K – 1:  8:20-1:50
Grades- 2 – 5:  8:35 – 3:05

Wednesdays: Dismissal at 1:50 for all grades

School is in session during these hours. Please be prompt to drop-off and pick-up your children. If arriving late, your child will be asked to obtain a late pass from the office. All gates are locked by 8:35 except for the mail entrance on Prairie Ave.

Drop Off:

8:00Drop off time is 8:00, unless a student is eating breakfast, then he/she can proceed to the cafeteria at 7:45.

Breakfast hours:

7:45 – 8:10 

Breakfast is free to all Miami-Dade County Public School students.

Lunch:
10:30-1:00

Lunch costs $4.20. For students approved for reduced-price meals, lunch costs $0.40.

Media Center Hours :

Our state-of the-art media center is open to all students during school hours. In addition, we are open after school until 3:30 for book return, check-out, and research.

Uniform Policy:

School Uniforms are Mandatory

  • Uniform shirts & jackets can be purchased on campus from the PTA
  • Students must wear closed toe shoes

Counseling:

Please call or set up appointments in the main office.

Counseling and advisement is available to students individually as well as on a small group and large group basis.

Immunization Information

Requirements for School Entry:

  1. A complete Florida Certification of Immunization Form DH 680 according to grade level.
  2. State of Florida School Entry Health Exam Form DH 3040 no older than 12 months.
  3. Tuberculosis Clinical Screening, PPD or Chest X-ray.

Immunization Guidelines for School Entry:

DTP or DTaP

  • Diptheria, Pertusis, and Tetanus or Diptheria, Pertusis and acellular Pertusis are generally given up until the age of 7. After 7, students are generally not given DTP because Pertusis has increased potential for side effects if given when the child is older.
  • Five doses of DTP are required. However, if the 4th dose was given after the 4th birthday, a 5th dose is not needed.

TD

  • If DTP cannot be given because the student is 7 years old or older, 3 doses of TD are required to complete immunization requirement.
  • If child has 3 TD’s, his immunization is complete for 10 Years. (TD is an adult dose

POLIO

  • Students are required to have 4 doses of POLIO unless they have their 3rd dose after their 4th birthday. If so, they only require 3 doses.
  • Students may have the POLIO vaccine until 18 years of age.
  • As long as a student is in school, even if they are over the age of 18, they may receive immunizations at the Health Department.

MMR

  • MMR is suggested not required. Two doses of measles are required.
  • If a student has one dose of measles to get into school, the second dose must be given on an accelerated schedule of 45 calendar days after the first.
  • If the student has one single measles and one MMR, delete the MMR date and use it as the 2nd measles, the 1st mumps, and the 1st rubella.
  • The fist measles or MMR must be given after the 1st birthday (or within 5 days before or after the 1st birthday) to be accepted by the M-DCPS computer.

HIB

  • HIB is not required. It does not have to be entered into the computer.

HEPATITIS B

  • HEPATITIS B can be given in a 3-dose series and in an accelerated 2-dose series. If the 2-dose accelerated series is given, the medical provider must indicate it in the Florida Certification of Immunization Form DH 680
  • There are generally 2 months between the 1st and the 2nd, and 3-4 months between the 2nd and the 3rd dose of HEPATITIS B.
  • If the 2-dose series is administered, enter 6 (000000) zeros in the third dose box and expire after graduation from high school in order for the computer to accept the entry.

VARICELLA

  • Student must have varicella vaccine or information on history of the disease.

EXEMPTIONS

  • There are only two types of exemptions: MEDICAL and RELIGIOUS. A Doctor must fill out the DH 680 form (Blue Card), part C for the medical exemption on the back of the card. The Health Department is the only one that can grant a RELIGIOUS exemption. The parent must call SIP (Special Immunization Unit) at 786 845-0550 to get the religious exemption.

School Entry Health Exam

DH3040 (Yellow) Physical Examination

The Florida School Health Services Plan provides that all children of school age shall have health examinations performed at appropriate intervals by their family physicians or by physicians provided by public health agencies. This health examination shall be administered within twelve (12) months prior to initial entry into a Florida school. Parents/guardians of all students are to present evidence of health examination on the appropriate School Entry Health Exam (DH 3040) prior to initial entrance into a Miami-Dade County Public School. Parents/guardians who, for economic reasons, are unable to have their children examined by a private physician may obtain a health examination from the Department of Health or neighborhood health centers.

To be considered complete, the school health exam must include a Tuberculosis and appropriate follow-up.

Students transferring into the Miami-Dade County Public Schools from another school within the State of Florida who have a completed health examination form as part of their school record need not be re-examined.

The School Entry Health Exam (DH 3040) shall be attached to the(DH 3041) Cululative Health Form and filed as part of the Cumulative Record Folder.

Florida Certificate of Immunization

DH680 Blue Card

  1. The Florida School Health Services Plan requires each child entitled to initial entrance into a Florida public school to present a Certificate of Immunization (DH 680 – Part A) from a licensed practicing physician or a county health officer prior to the child’s entry into school.
  2. Children admitted to school with temporary certification, Certificate of Immunization (DH 680-Part A-1 or A-2), shall be excluded from school attendance if additional certification is not presented on or before the expiration date noted on the temporary certificate.The principal is responsible for monitoring the status of students admitted with temporary certification. Two weeks prior to the expiration date, letters should be sent to parents/guardian alerting them to impending exclusion.
  3. The Plan further provides that any child shall be exempt from immunization upon written request of the parent/guardian stating an objection to such immunization on religious grounds, Religious Exemption Certificate (DH 681), or upon written certification, Certificate of Immunization (DH 680 – Part C), by a competent medical authority. It is understood that children with medical or religious exemptions will be subject to exclusion from school during any communicable disease epidemic.
  4. Each student is required to have on file a Florida Certificate of Immunization (DH 680) or an exemption for attendance in a public school in Miami-Dade County for grades Pre-K through 12. Students who do not meet this requirement will be subject to temporary exclusion from school until proper documentation is presented.

The Certificate of Immunization shall be attached to the Cumulative Form (DH 3041) and filed as part of the student’s Cumulative Record Folder.

Tuberculosis Clinical Screening
Each student shall have proof of a tuberculosis clinical screening and appropriate follow-up prior to initial enrollment in any grade in a Miami-Dade County Public School. This screening is to be administered at the time of the Student Health Exam and within twelve (12) months prior to initial enrollment in any grade in a Miami-Dade County Public School. If the screening indicates that a follow-up skin test is needed, a student can be admitted but only with a health provider’s statement that the student is free of communicable tuberculosis and can attend school.
